Job Summary
As a member of an interdisciplinary team, the Licensed Practical Nurse (LPN I) provides quality nursing care to clients of the Family Care Team, providing services to residents in the Bonavista region, with potential service to alternate sites using a hub and spoke model. The principles of the Family Care Team align with the patient health home model of care; using a collaborative health care team approach, from various health disciplines, in the provision of primary health care services.
Responsibilities include clinic care coordination and support to providers and patients, new patient intake assessments and populating new electronic medical records for new patients. Clinic screening and vital sign, point of care testing, venipuncture, wound care and other related clinic duties as required within the scope of practice of an LPN. The incumbent may be trained in the BETTER or similar approach to chronic disease prevention and screening to offer chronic disease prevention and screening services in a clinic setting, or will complete the training by a date designated by the manager. Duties may include education, consultation, motivational interviewing, assessment, investigation, follow-up, and evaluation within their case load. This position will require a highly trained individual who works well on collaborative projects and who has a comprehensive medical background.
Flexible hours are required (Combination of days, evenings, and weekends required).
This position reports to the Regional Primary Health Care Program Manager responsible for the service.

Regularly requires physical exertion and handling, moving, pushing or pulling objects between 10 - 15 lbs and occasionally required to support, lift or move patients/objects up to 30 lbs.
Regularly required to work in awkward cramped positions (bending/kneeling/stretching) and constantly standing and moving about.
Regularly pushes and pulls objects using light to medium forces and stands and walks.
Regularly requires visual concentration to give medications, read orders/referrals, patients' charts/document, observe and treat patients combined with time pressures and deadlines.
Regularly stands and walks, and drives to and from sites
Job Qualifications
Successful completion of an approved Licensed Practical Nursing program is required.
Current licensure with the College of Licensed Practical Nurses of Newfoundland and Labrador (CLPNNL) is required.
Current BLS certification is required.
Demonstrated strong interpersonal and communication skills.
Ability to establish and maintain effective working relationships as a member of an interdisciplinary team.
Ability to be self-motivated and able to work both collaboratively and with a high degree of independence and initiative.
Family practice clinic experience is an asset.
Proficient in computers/technology programs including, but not limited to, remote patient monitoring, telehealth, and the Microsoft Office suite (Word, Excel, PowerPoint).
Exhibited strong oral communication skills and the ability to deliver effective presentations in an adult learning environment.
Ability to organize and prioritize work and balance multiple activities concurrently.
A valid Newfoundland and Labrador class 5 driver's license and use of a private vehicle are conditions of employment.
A satisfactory record of work performance and attendance is required.
*The successful candidate must be willing to complete Electronic Medical Record (EMR) training upon commencing the position.
*The successful applicant may be required to complete the BETTER program practitioner training. If required, it must be completed at the first available opportunity, as determined by the manager.
