Job Summary
Eastern Urban Zone Primary Health Care and Chronic Disease Prevention and Management Program is seeking a Clinical Physiotherapist II to join the Comprehensive Respiratory Care Program (CRC) as an integral member of the multidisciplinary team. The role works closely with the CRC team and leadership, specialists, primary providers, programs across the organization, and community partners to deliver services.
Supported by the Division of Respirology, the CRC program is an evidenced based, innovative, pragmatic strategy to improve and promote lung health. It provides improved access to specialized care and acts as a bridge between acute and community care for patients living with respiratory disease. The program has several components including the INSPIRED COPD Outreach Program, which has demonstrated both marked reduction in ER visits and hospitalizations, and significant deferral of healthcare expenditure. The second component is the Comprehensive Respiratory Education Clinic where respiratory assessments diagnostic testing are performed for those living with asthma and COPD. Additionally, Education and self-management toolkits are provided by Certified Respiratory Educators for patients living with asthma, COPD, interstitial lung disease, neuromuscular disease (ALS). Furthermore, the program offers the Ottawa Model Smoking of Cessation as a service. Most recently, Pulmonary Rehab is a component of the CRC program. The CRC program offers both in person and virtual services to patients.
The role of the Clinical Physiotherapist II is to offer Pulmonary Rehabilitation Program by working with adult clients requiring pulmonary rehabilitation and prehabilitation. Clients may require pulmonary rehabilitation for a variety of lung diseases including chronic obstructive lung disease (COPD) or pre and post-operative lung transplant. Prehabilitation and risk factor modification is required for individuals identified to be at respiratory risk and aims to support individuals towards achieving optimal health outcomes.
The Clinical Physiotherapist II will be responsible for developing and implementing pulmonary rehabilitation and prehabilitation programming for clients, both virtually and in-person. The Clinical Physiotherapist II will also deliver and/or provide leadership for service delivery in therapeutic exercise prescription and risk factor modification for clients requiring pulmonary prehabilitation, rehabilitation and others, as determined by the program criteria. The Clinical Physiotherapist II is responsible for consulting and advising staff and other team members, providing leadership in program development and planning, provision of patient support, teaching, advanced assessment and treatment, as well as provincial program development, leadership and consultation within the provincial rehabilitation program.
In addition to clinical duties, it is a role expectation that the Clinical Physiotherapist II will lead program development and planning, preceptor students, provide mentorship and enhancement opportunities for staff in the practice area, collaborate with others for program issues and participate in activities of the program and the profession that contribute to a high quality, safe and healthy workplace and environment.
Travel throughout NL Health Services will be required. The PT II is required to work a flexible work agreement to meet the program needs.
This position reports to the Regional Manager, Primary Health Care and Chronic Disease Prevention and Management Program, and maintains accountability to the Professional Practice Consultant for Physiotherapy for professional integrity and practice issues.
